We provide Support Coordination, Groups, Community Access and Accommodation for people with disabilities across the Perth metro area.
What kind of Support Worker are we looking for?
- Motivated, understanding compassionate and energetic support workers who genuinely care about participants the way we do, and who want to work within Community Access to support our participants to achieve their goals and build their independent living skills
- You will have minimum 2 years previous experience in Community Access and/or Group Activities
- You must have demonstrated experience supporting adults and children with disabilities who may have complex needs and/or challenging behaviours
- We require our Support Workers to be available, flexible and committed
- You will have strong verbal and written communication skills
- You will have the ability to work as part of a team, however, can work autonomously in a professional manner
Your role includes, but is not limited to:
- Supporting our participants:
- in the community – to achieve their goals and build their independent living skills
- in Group Activities – as a support or as a Leader of one of our Groups
- Assistance with daily personal care, medication management, while following Allied Health support plans
- Documenting and accurately recording shift notes into relevant systems, incident reporting, and identifying areas of improvement
- Maintaining positive working relationships with participants and your team members
